New Approaches to Improve Treatment Outcomes of Vasculogenic ED

BACKGROUND & PURPOSE

The aim of this presentation is to provide an advice how to select and treat the candidates for vasculogenic ED treatment by Low intenstity Focused Extracorporeal Shockwaves (LiESWT) and how therapeutic approach could significantly improve the treatment results.

METHODS

We have developed a new treatment technique (LSTC-ED) that covers whole erectile tissue equally as well as the unique and first of its kind algorithm for tailor-made treatment which takes into consideration many factors that could have an influence on the treatment results. We consider very important proper evaluation of patient´s endothelial dysfunction preferably by the measurement of microalbuminuria and Ankle-Brachial Index (ABI). From September 2017 to March 2018 we have evaluated the treatment outcomes in a single-blind, sham-controlled, clinical trial with 60 patients randomised into 2 age-matched groups, Group A: treatment; Group B: placebo.

RESULTS

We have been able to confirm that this newly developed comprehensive approach with an emphasis on complete tissue coverage and tailored treatment could be benefitial and could significantly improve treatment outcomes. A statistically significant difference between these 2 groups was found at 4 and 12 weeks after treatment with regard to the quality of erection as measured by the IIEF-5, the EHS, SEP 2, SEP 3 and patient’s and partner’s satisfaction.

CONCLUSIONS

We could conclude that the complete tissue coverage and algorithm-based tailored approach, together with a proper patient´s endothelial dysfunction evaluation (microalbuminuria and ABI measurement ) are very important factors for successful shockwave treatment of vasculogenic ED.
